      The Sleep-Wake System is thought to be regulated by two major processes. One that promotes sleep (Process S) and one that maintains wakefulness (Process C). the need for sleep grows throughout the day and peaks just before you sleep while gradually decreasing during sleep. Process C is regulated by the circadian rhythm and counteracts the Process S, promoting wakefulness and alertness. Without Process C, the total sleep required remains the same but is randomly distributed throughout the day rather than separated into sleep and wake episodes.

      This refers to the daily rhythms in physiology and behavior. Circadian rhythms control the sleep-wake cycle, physical activity, food consumption, body temperature, heart rate, muscle tone, and hormone secretion. Structures in the hypothalamus of the brain are responsible for circadian rhythms. Animals appear to have an internal clock to help determine daily behavior and rhythms but the external day-night cycle is also largely influential on these behaviors. In mammals, there are specific genes that are essentially turned on and off in a cyclical pattern that is very close to 24 hours (1).

Older adults do show a decrease in melatonin levels. Also, as we age, we are actually more easily awakened during Non-REM sleep as well, which means our sleep is more easily disrupted by minor environmental changes.

      . When sleep is disrupted, levels of norepinephrine and epinephrine increase indicating activation of the sympathetic nervous system.   Long-term Sleep Deprivation

      Long-term sleep deprivation can significantly alter hormone production and release. Chronic sleep disruption is associated with adrenocorticotropic hormone (ACTH) and cortisol. Chronic sleep deprivation is also associated with decreased insulin sensitivity, therefore increasing your risk of type 2 diabetes. Decreased leptin and increased ghrelin can lead to increased appetite. In addition, sleep disruption leads to changes in pro-inflammatory cytokines such as tumor necrosis factor (TNF) and C-reactive protein (CRP). Higher sleep disturbance scores have also been associated with higher risk of elevated cholesterol, higher body mass index (BMI), and increased risk of hypertension. There is also evidence that long-term sleep deprivation can increase risk of certain types of cancer and even overall risk of death. (2)

      There are many prescription medications used for sleep. In the interest of time, I will only mention the most common prescription medications used for sleep.

        Benzodiazepines

      There are several types of benzodiazepine medications and some have been used for the treatment of sleep and anxiety for decades. These medications bind to and activate the GABA receptor. They all have sedating effects. They all also have significant addictive and abuse potential. In addition, these medications can interact poorly with alcohol leading to dangerous changes in blood pressure and breathing.

      Benzodiazepine Receptor Agonists

  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
                      
                      
    
    
      Zolpidem (Ambien), and eszopiclone (Lunesta) were developed in order to selectively bind to certain GABA receptors to help induce sleep while attempting to minimize the abuse and dependence potential of the benzodiazepine medications. These medications are very effective but still have significant risk of adverse effects. My recommendation is that these medications should only be used for short periods of time to help with short-term insomnia. The biggest potential adverse effects of these medications are that they can persist into the next day and can lead to potentially dangerous situations such as driving while still under the effects of these drugs. In 2013, the FDA recommended that clinicians use lower doses of zolpidem due to blood levels staying elevated the next morning.

  Sedating Anti-depressants

      Several older anti-depressants have sedating effects. The agents most commonly used for sleep are: Trazodone, Amitriptyline, Doxepin, and Mirtazapine. These medications have common adverse effects of dry mouth, dizziness, and next day drowsiness. I personally prescribe these medications only in limited cases such as patients who have failed other agents or those who have depression.

      Many antihistamines have sedating properties and are commonly used as sleep aids. Diphenhydramine is one of the most common OTC sleep aids and is found in brand name products such as Nyquil ,Z-quil, Tylenol PM, Advil PM and many others. Doxylamine is found in Unisom products. Unfortunately, these products are frequently overused, can rapidly lead to tolerance, and carry several possible adverse effects. My opinion is that these agents should never be used as sleep aids. These medications can reduce the quality of sleep, cause residual drowsiness the following day and can also cause dizziness, dry mouth, constipation, confusion and other adverse effects. Older adults are even more susceptible to these side effects and antihistamines can lead to falls during the night. Again, myself and most experts recommend against using antihistamines for sleep. My main issue with this is that, for most people, a physiologic dose of melatonin would be in the 0.3-1.0mg range but most supplements contain doses 10x higher in the range of 3-10mg of melatonin, which leads to elevated melatonin levels the following day and resulting drowsiness.       As we have discussed, sleep deprivation can lead to detrimental changes in memory and cognition, mood, and even increase the risk of depression, diabetes, obesity, heart disease and death over time. My preference is to avoid prescription sleep aids when possible and certainly to avoid antihistamines such as Benadryl (Tylenol PM etc.). I also prefer to keep the melatonin dose as low as possible. We are constantly researching new and safer sleep aids as there can be a lot of individual variation in the response to these supplements.   What is Epigenetics?

      While genetics are certainly important, the way your genes are expressed is really how they affect your overall health. One way to study this expression is called epigenetics. Put simply, epigenetics is the study of how your genes can be turned on or off by external factors. Consider that our cells all have the same DNA, but our bodies contain many different types of cells. This occurs because they have certain sets of genes that are expressed (aka “turned on”) and other sets that are inhibited (aka “turned off”). Think of your DNA as a script to a movie. The words represent the DNA base sequence, the script the DNA, and the actors are the cells. Think of epigenetics as the director. The director can tweak or eliminate certain parts of the script, making a significantly different scene. Imagine if Quentin Tarantino would have directed E.T. rather than Steven Spielberg, it would have been a completely different movie, right?       Epigenetics determine a cell’s specialization. As a fetus develops into a baby through gene expression (active) or gene silencing (dormant). However, environmental stimuli can also cause genes to be turned on or turned off. In fact, a study by the American Physiological Society suggests that maternal exposure to environmental pollution such as lead may affect the asthma risk of offspring for three generations! Some of these actions happen quickly while others take years. Additionally, certain diseases such as cancer can turn genes on or off. Epigenetics can be changed, whereas your genetics are there to stay. It isn’t just your epigenetics affected by your environment and choices though.

      This is currently a vigorously pursued topic of interest. If few could control how genes are turned on and off, we could potentially cure cancer, slow aging, and eliminate countless diseases. Although this seems reasonable, don’t get too excited just yet. In order for this therapy to work, the epigenetic treatments would have to be selective to irregular cells.   What are Wrinkle Relaxers aka “Toxins”?

                    Although Botox is the most well-known wrinkle relaxer, there are a total of four FDA approved brands in the U.S.: Botox, Dysport, Xeomin, and Jeuveau. To make things simpler for this blog, I will now refer to all of these brands collectively as “wrinkle relaxers”. Although they are somewhat different in shape and size, all of these brands are derived from a toxin produced by the bacteria clostridium botulinum. They also all work through the same mechanism shown below. These wrinkle relaxers actually prevent contraction of the targeted muscle. Without the target muscle contraction, you no longer make the wrinkles form in the skin. Over time, less folding of the skin allows the wrinkles to improve. Most of these wrinkle relaxers last 3-4 months so best results are with 3-4 treatments per year.

  Which Wrinkle Relaxers do I use most?

                    Although I am comfortable with and have patients request all U.S. brands, I use more Botox and Dysport in my practice. Most of my patients get very similar results with all of the brands but in my experience the key difference is duration. One brand may work slightly better or last longer in a particular patient. Most patients get 3-4 months duration, but this can vary from patient to patient.

  What are Fillers?

                    Fillers are prescription medications used for the treatment of volume loss and wrinkles/folds of the face. The vast majority of fillers are made of hyaluronic acid (aka HA). However, the size, shape, and linkage of the molecules varies between brands and indications. There are three common brands of HA fillers in the U.S.: Juvederm (Allergan makes Juvederm fillers and Botox), Restylane (Galderma makes Restylane fillers and Dysport), Belotero, and Versa. There are other types of fillers less commonly used. Radiesse, which is made from calcium hydroxyapatite a chemical found in your teeth and bones, is used in the chin, cheeks and hands mostly. Sculptra is made from Poly-L Lactic Acid and stimulates your body to produce more collagen at the site of injection. Sculptra is mostly used in the cheeks.

  Which Fillers do I prefer?

                    This question is much more difficult to answer than my favorite wrinkle relaxer. My filler choice varies too much based on where I am injecting the filler as well as patient specific qualities such as age, degree of volume loss, prior history, treatment goals, and budget. I honestly carry most every major type of filler in my clinic as they each have advantages and disadvantages. For instance, I use seven different fillers in the lips and seven different fillers in the cheeks.

One good analogy is to think of your face as a bed with the sheet being the skin and the mattress representing the underlying structures such as collagen, fat, muscle, and bone. Wrinkle relaxers keep the sheet from folding by relaxing muscles whereas fillers plump the mattress by adding volume.

Most cleansers aimed toward acne have some combination of benzoyl peroxide, glycolic acid, or salicylic acid. Cleansers must be used consistently to see results. Benzoyl peroxide acts to kill bacteria on the skin.  Acetone can help to reduce sebum on the skin and alcohols can help to kill bacteria.

      Diindolylmethane or DIM is a chemical found in cruciferous vegetables such as broccoli, cauliflower, and Brussel sprouts. There is a fairly large amount of research on DIM, mostly evaluating the reduced risk in certain types of cancers such as breast or uterine cancer (1). DIM alters the metabolism of estrogens so that there are less harmful metabolites and more protective estrogen metabolites. DIM has also been used extensively to improve hormone related acne. I recommend and carry DIM from Douglas Laboratories. They formulate their DIM in a way that is absorbed much better than most other companies (2). I also recommend Accumax from Jane Iredale, which contains DIM and other nutrients such as vitamins A, C, and E, that have been shown to improve acne over time (4).

A physician or aesthetician can uncover these lesions in a sterile technique to minimize the risk of scarring and spread of acne lesions. It is also a good idea to clean and replace makeup brushes regularly. While it is not completely understood, it is likely a good idea to use makeup free of parabens, alcohols, fragrances, dyes, silicones, and acrylics.       Stress Urinary Incontinence (SUI) is extremely common in women who have had children. Carrying and delivering a baby puts significant stress on the muscles of the pelvic floor and vaginal region. SUI is the leaking of urine with physical activity/straining or with sneezing/coughing, jumping. Stress urinary incontinence occurs when the muscles and other tissues that support the bladder weaken and when the tissue of the pelvis becomes thinner and less supported. When those muscles weaken, anything that exerts force on the abdominal and pelvic muscles (sneezing, coughing, lifting, laughing etc.) can put pressure on your bladder and cause leakage. Although heat can help tighten skin, cold is much better than heat for reduction of fat. Fat cells can be frozen without damaging surrounding cells. It is much more difficult to safely damage fat cells with heat because the skin can burn at temperatures lower than that required to damage fat cells.
